Blood Tests

Requesting a blood test 16 years and over

Have you been asked to have a blood test by either a member of staff at the Surgery or a clinic you have attended?

Yes: If you have been asked by the hospital or a clinic to have the blood test, have they provided you with the form?

No: If you have not consulted a health care professional, you need to do so prior to booking your appointment. This is to ensure that a blood test is the right investigation for you and if it is, that all the appropriate tests are requested.

Have you been given the form?

No: Please ask the clinic to provide you with the form or ensure that the Surgery has received the letter that details what investigations are needed.

Yes: Please bring the form to your appointment. If the Surgery has requested the form, the form will be on your records for you.

Please be aware if your blood test is fasting or non-fasting. If you are unclear, then please ask.

How do I book a blood test and where can I have my blood test taken?

  • Book online via the link on our website
  • Book in person at reception or by calling our telephone appointments line on 023 8044 8901

We may not always have the availability of appointments to suit your needs. If this is the case, you may be given alternative options to book your test:

  • Via one of lour local Hub sites. This can be booked via reception. Please ask for a copy of your blood form to take to the appointment.
  • You can book an appointment directly with University Hospital Southampton via 023 8120 4877 or patientbooking.uhs.nhs.uk

Please ensure you have had at least a glass of water prior to your appointment, including all fasting appointments. Failure to do so can make your veins harder to find, or affect your kidney function, which may mean your blood test will need repeating.

Requesting a blood test aged under 16 years

Surgery Request: Please ask either the clinician or member of the reception staff to provide you with the blood form.

Hospital or another Clinic: Please ask the clinic to provide you with the form or ensure that the Surgery has received the letter that details what investigations are needed.

Butterfly Room: Blood tests for children are taken at University Hospital Southampton. You can book your appointment via calling 023 8120 2024 (Monday, Tuesday, Wednesday: 8.15am to 4.45pm; Thursday: 10.00am to 4.45pm). Please remember to take your blood form with you.
