MAKE
AN APPOINTMENT?
Routine appointments with the doctors may be made in person or by telephoning
023 8044 8901 between 8.30am to 1.00pm and 2.00 to 5.00pm, Monday to Friday.
You can also use our automated telephone system which is available 24/7.
• Please make separate appointments for each person who needs to see the doctor.
• If you are unable to keep your appointment please let the surgery know as soon as possible.
• Although you are registered with a certain doctor you are able to make an appointment with any doctor in the practice.
We operate a triage system each weekday morning and Tuesday, Wednesday
and Thursday afternoons. This is for patients requiring an urgent appointment
and allows you to speak with the nurse who will give the appropriate advice and/or
appointment.

OBTAIN
HOME VISITS?
Home visits are undertaken for patients unable to get to the surgery.
These are generally very elderly and infirm and terminally ill patients.
The decision to visit at home is at the discretion of the doctor who
may telephone to check on the patient's condition before visiting.
The telephone number for home visit requests is 023 8044 8558. Requests should
be made before 11.00am on the day the visit is required.

MAKE A COMMENT/COMPLAINT?
We are very happy to receive
constructive comments and suggestions for improving our service to
patients. Similarly, if you have a complaint we will deal with it
in a constructive way.

HOW DO I...
ORDER A REPEAT PRESCRIPTION?
If you are on regular medication, a printout of all your current medication
accompanies each prescription.
When you require a repeat prescription return the printout with the medication
you require ticked.
Repeat prescription requests can be made in person at the surgery or by post.
Telephone requests will not be accepted to avoid possible errors. We are happy
to post your prescription to you if you provide us with a stamped addressed envelope.
PLEASE ALLOW TWO FULL WORKING DAYS FOR THE PREPARATION OF YOUR REPEAT PRESCRIPTION.

HOW DO I...
REGISTER WITH THE PRACTICE?
If you wish to become a patient at the surgery you will be required to fill
in some forms giving us information about yourself and your previous medical
and family history. You will also be offered a new patient health check with
the practice nurse.
UPDATE OF PATIENT
INFORMATION
To ensure the practice holds the correct information about you, it is very
important that you inform us of any changes eg name, marital status, address
or telephone number.
